Maison Caracol is the community-based social pediatrics center in Pierrefonds-Est. It provides integrated social medicine, pediatrics, social work, and legal services as well as quality professional services and comprehensive development activities to children aged 0-17 years in accordance with their rights and interests.
Caracol House provides care and services to children who live in a vulnerable context due to low family income, precarious immigration status, health problems, behavioral problems and learning difficulties. Maison Caracol has been in existence for 5 years and currently follows more than 400 children who are experiencing difficulties in their life trajectory. Caracol House is a community-based organization that is committed to the well-being of children and their caregivers.
Job Description:
The incumbent participates in various interventions with vulnerable children and families in the community to ensure the development of their full potential. He/she provides consultation, support and referral services in collaboration with an interdisciplinary team from the Community Social Pediatrics Centre and in partnership with other social support services in the community.
Responsibilities:
- Development and implementation of intervention plans: Actively participate in the determination of needs and appropriate services during assessment/referral meetings, in co-intervention with the physician.
- Intervention and follow-up: Assume responsibility for the psychosocial assessment, the development of the intervention plan and the individualized service plan.
- Transmission of information concerning the children’s file: Write psychosocial reports as recommended by the Quebec Order of Social Workers for each of the children’s physical files.
- Collaboration with the various partners: Ensure follow-ups in an interdisciplinary context and make sure that there is a concerted effort on the interventions to be carried out with the children.
- Work in close collaboration with family, social and institutional networks.

Requirements:
- Bachelor of Social Work
- Member of the Ordre professionnel des travailleurs sociaux du Québec
- Knowledge of intervention with vulnerable children and families.
- Knowledge of the characteristics of child development.
- Sensitivity to intercultural realities
- Fluency in French and English (written and spoken)
- Mastery of Microsoft Office software
